Admissions

Admission Criteria

• Individual must be at least 18 years of age
• Individual must have a primary diagnosis of intellectual disability that may also include a concurrent psychiatric condition
• Individual must have an established need for support and have limitations in adaptive skills as determined by the Level of Functioning assessment in two or more of the following areas:
  • Socialization
  • Personal Care
  • Communication
  • Money Management
  • Community Integration
  • Use of Community Resources
  • Health and Safety
  • Mobility
  • Functional Academics
  • Behavior Management
• Are eligible for Medicaid Waiver through a community services board or private funding
• Have the ability to benefit from services provided
• Have clinical and medical needs that can be accommodated by Assured Development
• Services, LLC personnel and/or contractors
• Have a current physical examination
• Be free from communicable diseases (ex. Tuberculosis)
• Have all admissions paperwork and physicals completed
